Real Results with Consultant Oculoplastic Surgeon Mr Michail Malandrakis

Before and after: brighter, more open eyes with a fully natural result.
Heavy, hooded upper eyelids don’t just affect how you look in the mirror – they can make you appear tired, older or even a little stern, no matter how well-rested you actually are. For men in particular, this is often a gradual change that creeps up with age as the brow drops and excess skin builds up on the upper lid, sometimes blending into the eyebrow itself.
These striking before and after photos show the results of an upper blepharoplasty (eyelid lift) performed by our Consultant Oculoplastic Surgeon, Mr Michail Malandrakis, at Cosmedics Skin Clinics in London.
The Transformation

Excess upper lid skin removed while preserving a masculine brow–eyelid relationship.
Looking at the before images, you can see the classic signs of upper eyelid ageing:
- Excess skin folding down over the lid, a heavier brow line
- Eyes that appear smaller and more closed off, even when fully open.
In the after photos, taken from the front, left and right angles, the difference is immediate.
- The eyes look brighter, more open and noticeably more youthful – without looking altered or “done”.
- The brow area appears lifted and the whole upper face reads as fresher and more alert.
What stands out most in this particular case is how natural the result looks. This is very much the Cosmedics approach: our surgeons are careful never to overcorrect, so results enhance the eyes rather than changing the character of the face.
What Is Upper Blepharoplasty?
Upper blepharoplasty is a minor surgical procedure that removes excess skin, and sometimes fat, from the upper eyelid. It’s one of the most effective ways to rejuvenate, lift and open up the eye area, producing more noticeable and longer-lasting results than non-surgical treatments alone.
At Cosmedics, the procedure is carried out as day case surgery under local anaesthetic, meaning patients are awake but feel nothing, and can go home the same day rather than facing a hospital stay. The incision is made along the natural crease of the upper eyelid, so any resulting scar is very well hidden once healed.
An Expert’s Approach to Male Blepharoplasty
Male eyelid surgery isn’t simply a smaller version of the female procedure – it calls for a distinct surgical approach. As Mr Malandrakis explains:
“Male upper blepharoplasty requires a slightly different approach from the traditional female procedure. While the aim in women is often to create a cleaner, more defined upper eyelid crease and a more open appearance, in men it is particularly important to preserve the naturally lower eyelid crease, fuller upper lid and masculine brow-eyelid relationship.
“Excessive skin or fat removal can create an over-defined or hollow appearance that may feminise the eye or make the surgery look obvious. The goal of male blepharoplasty should therefore be conservative: removing the heaviness and excess skin that create a tired or aged appearance while maintaining sufficient upper-lid fullness and respecting the natural position of the male brow.
“When carefully planned, the result should make the patient look fresher and less tired without looking noticeably ‘done.'”
— Mr Michail Malandrakis, Consultant Oculoplastic Surgeon, Cosmedics Skin Clinics
This conservative, gender-aware approach is exactly what’s reflected in the results above: the eyes look brighter and more rested, but the natural masculine shape of the eyelid and brow has been fully preserved.
Why More Men Are Choosing Eyelid Surgery
While blepharoplasty has traditionally been associated with female patients, it’s an increasingly popular procedure among men who want to look as refreshed and alert as they feel. Common motivations include:
- Heavy or hooded eyelids that make the eyes look permanently tired
- Excess skin that has started to affect the upper field of vision
- A desire for a subtle, natural-looking rejuvenation rather than an obvious “surgical” change
- Wanting results that complement a masculine facial structure, not soften it
Because the procedure is performed by an experienced oculoplastic surgeon, results like these are tailored to keep the outcome looking authentically like the patient — just fresher and more youthful.
Recovery
One of the advantages of the technique used at Cosmedics is a comparatively quick and manageable recovery. Most patients experience some bruising and swelling for a few days, with stitches removed around a week after surgery, and many people are back to their normal routine within a few days to a week.
